Fill out a Kaiser Permanente Washington Medicare Advantage (HMO) election form to enroll in one of our health plans (effective through December 31, 2020). You can also use this form to add optional dental benefits to your current coverage.

There are limits on when and how often you can change your Medicare coverage:
The initial enrollment period is 3 months before, the month of, and 3 months after your 65th birthday (or otherwise first become eligible for Medicare).
From October 15 through December 7, 2019, anyone with Medicare will have an opportunity to switch from one Medicare plan to another. Coverage is effective January 1, 2019.
From January 1 through March 31, 2020, anyone on a Medicare Advantage plan may disenroll and move back to Original Medicare or enroll in another Medicare Advantage plan. You may choose to enroll in a stand-alone Part D prescription drug plan when you move back to Original Medicare during this period.
Switching from one Kaiser Permanente Medicare health plan to another counts as making a change. Generally, you can't make any other changes during the year except under special circumstances, such as if you move out of the plan's service area or are eligible for Medicaid coverage.
